SENSITIVE TOOTH: WHITE FILLINGS
A regular dental check-up is best to see if you need a new filling or if your existing filling should be renewed. But if you feel a sensitivity on your tooth it means it's time to visit your dentist. If your tooth has hot, cold or sweet sensitivity this can be because of a decay in dentin level. Decay should be cleaned first and then a white composite filling will be placed to the cavity.
Sometimes, a tooth may deteriorate as a result of decay or issues with the root canal and may need to be restructured as a result. In such cases, the tooth is measured using digital technology, a replacement section is prepared from porcelain or fiber-reinforced composite materials in the laboratory and then applied with special adhesives into the tooth. Inlay-onlay fillings are designed to take account of contact with teeth on either side, form, hygiene and function.
Depending on the size of cavity, it varies however, white fillings take half an hour and cost 60 GBP per tooth. In case of a mercury removal or cavity sealer operation, it takes more time and material, which effects additional costs with the range of 10-20 GBP.
PAINFUL TOOTH: ROOT CANAL TREATMENT
If you have a constant pain or a pain which is waking you up at night timei this means that your decay is at pulp level. This requires a root canal. The nerves of the tooth will be taken out and the root canal will be filled with root filling.
The rest of the tooth will be restored or rebuilt by a white filling or crown.
Root canal treatment costs change due to number of roots treated. Anterior teeth usually have 2 roots while posterior teeth usually have three roots. Total cost will change between 150 GBP and 200 GBP per tooth. White filling over the tooth costs 50 GBP in addition to root canal treatment. At the end of the treatment your natural tooth will be saved, which is a better (and of course budget friendly) option than having an implant.
Each patient and each tooth is different but mostly, one visit will last 60-90 minutes and you will need one or two visits depending on your need. Your dentist can inform you better during your consultation.
What is endodontic treatment?
Endodontic treatment, also named as a root canal treatment is a procedure we carry out to save a tooth from extraction. It is performed mostly when a patient has severe pain or infection, sometimes in the form of swelling or abcess and sometimes symptomless but diagnosed with an x-ray.
When do I need a root canal?

If there is swelling or a continuous discomfort in your chewing function, you might want to get your tooth checked.
Is root canal treatment painful?
Root canal treatment begins with an injection administered by your endodontist, therefore numbing the tooth which reduces the risk of discomfort for the patient considerably. In some cases, more than one injection may be required but, thankfully, the days when it was a painful procedure are long gone. Your dentist will make sure you are pain-free during the whole procedure.
Should I use antibiotics beforehand?
Antibiotics are only really required to help patients with infection-related issues. If you are suffering considerable pain and/or swelling, your dentist may prescribe an antibiotic to help clear up any infection, once the issue has been identified and addressed. For example, the pain may be caused by inflammation around the site of previous root canal work, making it difficult to chew. Facial swelling can also be caused by a fistula (an abscess or ulcer) around the gingiva (the gums). Sometimes, treatment can involve an abstraction and antibiotics would be prescribed to deal with any infection but they are not used as part of routine root canal procedures.
If my root canal treatment was not completed perhaps because of Covid-19 travel complications but I'm not in a pain, would it be a problem?
We would always recommend prompt and complete treatment, even if the patient is not feeling any significant pain. Often, the reason for a lack of any immediate pain is that the nerve has died, leaving a cavity ideal for bacteria which can lead to discomfort later on. Soon, the patient may not be able to chew properly or may suffer facial swelling which may result in even more extensive treatment later on.
Is it necessary to have a crown after root canal treatment?
If the tooth is being involved in root canal treatment can be restored with only a filling afterwards, we would discuss options with the patient. However, it’s important to note these teeth are often already damaged, decayed or fractured. Repair may therefore involve a very large filling or the construction of root supports (fiber posts, etc.) which will help to ensure chewing forces evenly distributed, ensuring the repaired tooth lasts longer. And your dentist will inform you whether you need a crown or not.
How about after the procedure?
When you leave the office, you may feel some discomfort in chewing for a few days. This is not uncommon and can be solved by a pain medicine prescribed by your dentist if necessary. Some infected teeth might take some more time to heal, please contact your dentist if you experience any unexpected pain.
If you already removed the nerves, why does it stil hurt?
Removing the infected pulp tissue eliminates the source of infection, but the resolving of the inflammation in the surrounding tissues take some time. Usually the body can do this by itself but sometimes antibiotics and antiinflammatory medicine is needed.

I am pregnant and I have a tooth-ache. Can anything be done?
As much as we would prefer to finish all dental treatment before pregnancy, yes, if you are pregnant and have a tooth that causes you trouble, there is still something we can do. Unfortunately we cannot take any x-rays, so please bring any x-rays you already have in hand, that can give us some information. We usually prefer to leave big operations after your pregnancy is over but if one tooth is keeping you from having a good nights sleep, an initial session of a root canal treatment can be performed, and after your childs birth, you can come for the completion of the treatment. We might ask for a consent from your gynecologist if needed.
I have pus coming out from a pimple like tissue right next to my tooth. What is wrong there?
This is what we usually call an endodontic fistula. An infection can show up in two ways, either in an acute version, like a swelling in the face; or in a chronic version like an endodontic fistula, which will be much less painful, less symptoms in most cases but usually comes with a lot of bone loss since it has been there for a long time. The infection in the tooth end tries to find a way to release itself and the fistula is the way. The treatment on the other hand, is the same. We just need more than a single session so that we have time to observe the infection and whether or not the fistula is closed. If the infection is there for a very long time and the tooth is non responsive to the treatment, extraction is an option to be considered.
One of my front teeth has a darker color than the other ones. Is this a problem?
This usually happens when a trauma occurs, like a fall from a bike, an unfortunate fist coming right at your tooth and this particular tooth loses its vitality. What happens here is, the nerve bleed inside the tooth, and just like any other place in our body, the blood inside changes color, like when you would bump your leg into a door and it would bruise, this is the bruising your tooth gets. Only, there is no system to let it out as in your leg. So the coloring stays.
Aside from the coloring, this is usually also an indication for us that shows that the tooth might not be vital anymore and therefore we check the tooth for any infection around the root. And if there is an infection, the treatment will be a root canal treatment.
Why does some treatments take longer than the others?
There are many criteria that affects the length of the treatment. Some of the tooth we come across can be treated in a single session, and some need more than one session. If the tooth is an infected tooth, the patient has come with a swelling or a chronic endodontic fistula we usually need to do more than one session treatments. Also sometimes vital tooth with no major infection might need another session if the patient is symptomatic or the dentist sees the need to leave a medication inside the tooth for another week, depending on several reasons, like bleeding in nerve end. Your dentist will be able to tell you more information on this depending on the specific tooth she is working on.
If our patient's tooth is sensitive to hot, cold or sugary foods, this may be a sign of caries at the dentin level.
What needs to be done is to clean the caries of the patient under local anesthesia, then put a white composite filling in the emptied cavity and restore aesthetics and functionality.

In our hospital, the composite filling process takes an average of 30 minutes for a tooth and is completed in a single session. When the number of existing decayed teeth of the patient is high, there may be an increase in the number of sessions in line with the needs of the physician.
In procedures such as 6-8 Filling, one side is finished in the first session and the other side is completed in the other session, so that the patient does not have a deficiency in chewing function between sessions.

It is the process of cleaning, shaping and filling this area as a result of the nerve chamber of the tooth being affected by deep caries, trauma, and sensitivity.
As a result of this treatment, the tooth will lose its vitality, but will continue to function. If this treatment is not done, the infection will cause tooth loss and more extensive treatments such as bridges or implants will be required.
A caries at the pulp (nerve tissue) level can be treated with the help of root canal therapy.

Clinical process of root canal treatment
The first diagnosis is made by intraoral examination and x-ray analysis.
In patients who are scheduled for root canal treatment, the infected nerves of the tooth are cleaned and the cleaned area is filled with canal filling.
The rest of the tooth is given aesthetics and function in the visible region with a composite filling or crown.
Root canal treatment is performed under local anaesthesia and the patient does not feel any pain or sensitivity during the treatment process.
Clinical duration of root canal treatment
Every patient and every tooth is different, but usually a visit takes 60-90 minutes and the treatment is completed in one or two sessions depending on your needs. The first appointment is given for 1 hour, since the patient who comes with pain will have a possible root canal treatment, 1.5 hour appointments are given by the doctor himself. Regarding the number of sessions, the dentist gives clear information to his patient when he completes the necessary controls and analyses.
Root canal treatment pricing
The number of tooth canals is different in anterior and posterior teeth. The anterior teeth have 1-2 canals, while the posterior teeth have 3-4 canals. That's why their pricing is different.
Re-treatment of teeth that have undergone root canal treatment in the past is called “Retreatment” and its fee is higher due to the larger size of the procedure.
Root canal treatment costs 225 GBP for a molar, 175 GBP for a front tooth. In addition, a white filling which costs 70 GBP will be performed over the root canal.
Post-treatment process
Some discomfort may be felt when chewing for a few days immediately after root canal treatment. This is normal and a dentist-prescribed pain reliever can be used if needed. Some infected teeth may take some time to heal. In case of unexpected pain, a control session can be taken from the dentist.
Sometimes, after caries or root canal treatment in our teeth, there is excess material loss in the dental tissue. It is not possible to treat these teeth with excessive material loss with fillings in the mouth, or when the teeth are cut and porcelain veneer, too much material is lost.
In such cases, the process of measuring the tooth with digital or classical measurement methods, preparing it from porcelain or fiber-reinforced composite materials in the laboratory and applying it with special adhesives into the mouth is called inlay-onlay filling process.
The contact, form, interface hygiene and function of inlay-onlay fillings with lateral teeth are ideally prepared in the laboratory. The application of inlay-onlay fillings into the mouth takes a shorter time than normal fillings.
